Why Attaching Forms to Locations

Attaching forms to locations connects data collection to the physical place where it happens. Staff can fill in a daily temperature check when they're in the kitchen, or complete a contractor sign-in when they arrive on site. When someone scans a location's QR code, they see any forms attached to that location—and submissions are automatically linked to that location. No need to remember which form to use or where to find it.

Before You Start

You need the Manager role for the Premises module to attach forms. Forms must already exist in your form library and be published. If you don't see Attach Forms, ask your site administrator to update your permissions in Settings > User Management.

How to Attach Forms to a Location

  1. Go to Premises > Locations and select the building or room

  2. Scroll to the Forms section on the location details page

  3. Click Attach Forms

  4. Browse or search for forms, then check the boxes next to each one you want to add

  5. Click Attach [number] Forms

Forms appear in the location's Forms section immediately. Staff can click any form name to fill it in.

How to Make a Form Publicly Accessible

By default, attached forms require users to be logged in. You can make specific forms publicly accessible via QR code scanning—useful for visitor check-in or contractor sign-in at the site entrance.

  1. Find the form in the location's Forms section

  2. Turn the Public toggle on next to the form

Anyone who scans the location's QR code can access and submit that form without logging in. Public forms must be enabled in your site's premises settings.

How Forms Work When Scanning QR Codes

When someone scans a location's QR code:

  • They see the location and any attached documents and forms

  • If the form is Public, they can submit without logging in

  • If the form is Private (default), they must log in first

  • Submissions are automatically linked to that location

Child locations (e.g. rooms within a building) inherit forms from their parent. You can attach additional forms directly to child locations if needed.

Common Questions

Can I attach the same form to multiple locations? Yes. A form can be attached to as many locations as you need.

Do I need to attach forms to child locations separately? No. Child locations automatically inherit forms from their parents. Attach location-specific forms directly to child locations.

Can I attach forms to assets too? Yes. Forms can be attached to asset types (e.g. all fire extinguishers) and individual asset instances. Open the asset or asset type and use the Forms section to attach forms, similar to locations.
