Create an incident

Report an incident from the CalmCompliance app or by scanning a QR code. Once submitted, the incident is recorded in the system, reviewed by AI if enabled, and triaged by a manager.

Before you start

To create an incident from the app, you need the Incident module member role or higher. Anyone can submit an incident via a QR code, with or without an account.

Report from the app

  1. Navigate to your site and open Service Desk > Issues, then select Report Incident. Alternatively, go directly to the Incidents module and select Report Incident.

  2. Enter the required details:

    • When did this incident occur? — required. Choose a date and time; it cannot be in the future.

    • Description — required. Write at least 10 characters and fewer than 1,000.

    • Severity — optional. Select how urgent the incident is.

    • Location — optional. Select one or more locations, scan a location QR code, or describe the location in free text.

    • Asset — optional. Select one or more assets, scan an asset QR code, or describe the asset in free text.

    • Images — optional. Upload photos related to the incident.

  3. Click Submit Incident.

  4. After submission, you will see Incident Reported Successfully! with options to View Incident or Done. If your site is configured to skip the success page, you are taken directly to the incident detail page.

Report from a QR code

Site admins can set up QR codes so anyone can scan and report an incident without signing in. There are three types:

  • Site-wide incident QR code — opens the incident reporting form for the site.

  • Location QR code — opens the form with that location pre-filled.

  • Asset QR code — opens the form with that asset pre-filled.

Point your phone camera at the code, tap the link, and fill in the form. Anonymous reports must include a location or asset and can optionally include contact details.

What happens after submission

Once you submit, the system records the incident and sends you a confirmation: Your incident report has been submitted and recorded in the system.

If AI is enabled for your organization, the system reviews the description and writes AI suggestions for category and severity into the incident record. A manager then reviews the suggestions and sets the final triaged values. If AI is not enabled, the system applies the default assignee instead.

Only managers and admins can see the investigation timeline and corrective actions. Members can view their own incidents and add comments.

Members can see incidents they are involved in and add comments, but they cannot view investigation steps or follow-up actions.
