View and edit a risk assessment item
Open an individual risk item from any assessment to view its full details, edit hazard fields in a draft, create follow-up tasks, add comments, and follow the item for notifications. Each hazard has a stable link that resolves across republished versions, so shared bookmarks remain valid after the assessment is updated. Assessments can now contain multiple Matrix Sections, each with its own register of hazards. A hazard belongs to the Matrix Section where it was added, and the assessment page rolls up scores across all sections.
Before you start
You need the Manager role or higher for the Risks module to edit, delete, or create tasks from a risk item. Member and higher roles can open the detail page, view hazard information, add comments, and follow items for notifications. If you do not see the options you need, ask your site administrator to update your permissions in Settings > User Management > Edit Access. For a full role overview, see Risks roles and access.
Open a risk item from an assessment
Go to Risks and open the assessment.
Find the hazard in the risk items list and click the row.
The page opens to Risk #n, with breadcrumb links back to Risks and the assessment name. If you are viewing a specific version, the breadcrumb shows the version label, such as Assessment Name Β· v2.1.
What the detail page shows
The main column displays the hazardβs fields and scores. The metadata grid shows the parent Assessment, Version, Risk Number, Initial Risk, and Residual Risk. The version label indicates whether the view is Editable in this view (a draft) or Read-only snapshot (a published or historical version).
Fields visible on the page include:
Risk Description β the hazard or scenario
Initial Risk Assessment β severity and likelihood before mitigation
Existing Controls β safeguards already in place
Mitigation Controls β planned actions to reduce risk
Residual Risk Assessment β severity and likelihood after mitigation
Risk Scores β calculated Risk Score and Residual Risk
People & Groups on a hazard
Each risk item has a People & Groups section that shows who is involved in or affected by that hazard. You can add participants directly from the item detail page when the assessment is in an editable draft. Published and historical versions show the section as read-only.
To add a participant to a hazard, open the People & Groups section, choose a participant type, select the person or group, assign a role, and add any optional notes. To remove a participant, use the remove action and confirm the dialog.
Participant counts also appear on the hazard card in the assessment list, so you can see at a glance how many people or groups are linked to each item.
Edit or delete a risk item
You can only edit or delete a hazard when the assessment is in an editable draft version. Published and historical versions are read-only.
On the risk item detail page, make changes to the Risk Description, controls, or matrix selections.
Click Save Changes.
To delete the item, click Delete Item and confirm in the dialog. Deletion is only available on editable drafts.
Create a follow-up task
Follow-up tasks for a hazard are created from the item detail page, not the assessment list.
Open the risk item detail page.
In the task section, click Create Task.
Review the pre-filled Title and Description. The title defaults to the risk description, and the description defaults from the mitigation controls.
Choose a Priority: Low, Standard, High, or Critical.
Under Assigned to Team, select the group responsible for the work.
Under Assigned to User, select an individual. If you choose someone outside the assigned team, a warning appears, but you can still proceed.
Optionally, set a Start Date for when the task should appear in the assignee's inbox, and a Due Date for when it becomes overdue if not completed.
Add any extra context in Notes.
Click Create Task.
The task is created and linked to the risk item. The assignee can complete it from their inbox or work order list.
Comment on or follow a hazard
Comments and following on individual hazards are separate from assessment-level discussion. You can follow a hazard to receive notifications about changes, and anyone with comment access can add item-level comments in the sidebar.
Open the risk item detail page.
In the sidebar, add a comment or click Follow to receive updates.
Assessment-level comments and follows remain unchanged on the main assessment page.
When a hazard is not in the selected version
If you open a risk item link for a version where the hazard was not yet added, the page shows Not in This Version instead of a generic not-found error. The message explains that the hazard may have been added in a later version. Comments and follows are still available in the sidebar.
Your options are:
Back to Assessment β return to the assessment page for the selected version
View Earliest Version β open the earliest version that contains this hazard
Share a stable link to a hazard
Each hazard has a stable link that survives republishing and version switches. You can copy the URL from the browser address bar and share it with teammates. When the assessment is republished, the same link resolves to the hazard in the new version. The item detail page reflects whichever version you are viewing on the assessment, and the editable or read-only state follows the version.
