Work with risk assessment sections
Risk assessments are built from ordered sections that you can add, edit, reorder, and delete. Each section is a content block: a Rich Text narrative, a Matrix Section hazard register, a People & Groups participant list, a Hazard Classifications multi-select hazard register, a Hazardous Materials linked register, a Linked Documents section that references published documents from the site library, an Image Gallery photo collection, or a Map section with an interactive Google Map. You control the layout and content of the assessment page by choosing which sections to include and in what order.
Before you start
You need at least the Member role for the Risks module to add, edit, reorder, or delete sections on a draft assessment. Published and historical versions are read-only. For role details, see Risks roles and access.
Section types
There are eight section types. You can use any combination and any number of sections on the same assessment.
Rich Text — a prose block with a title, optional description, and a rich-text body. Use it for introductions, scope statements, or context.
Matrix Section — a hazard register with its own title, description, and a list of risk items scored against the assessment matrix. An assessment can contain multiple Matrix Sections, each with its own hazards.
People & Groups — a participant list with a title and description that records who is accountable for, exposed to, or involved in the assessment. It shows both assessment-wide and hazard-level participants.
Hazard Classifications — a multi-select list of standard hazard classifications with optional notes. Use it to record which hazard categories apply to the assessment, displayed with pictograms and names.
Hazardous Materials — a linked register that connects to hazardous material records. Each linked row shows the material’s current name, description, and hazard pictograms. It stays live: if the material is updated in the register, the assessment reflects the change. This section type appears under the Linked records group in the section picker.
Linked Documents — a live reference to one or more published documents from the site document library. Each row shows the document name, identifier, version label, and type icon. If a document is renamed or updated in the library, the assessment reflects the change without relinking. This section type appears under the Linked records group in the section picker.
Image Gallery — a photo collection with a title, optional description, and a grid of uploaded images. Use it to attach photos, diagrams, or evidence to the assessment.
Map — an interactive Google Map with a saved view, an optional pin, and an optional pin label. Use it to show a location, site entrance, or hazard area. It appears under the Basic group in the section picker.
Add a section
Open the draft assessment.
Click Add Section.
In the picker, choose Rich Text, Matrix Section, People & Groups, Hazard Classifications, Hazardous Materials, Documents, Image Gallery, or Map. Hazardous Materials and Documents appear under the Linked records group. Map appears under the Basic group. You can use the search box to filter the list.
The section is added to the bottom of the page. Fill in the Title and optional Description, then click Save.
Edit a section
In the section header, click the edit control.
Update the Title and Description.
Click Save. Click Cancel to discard changes.
Reorder sections
Use the Move Up and Move Down controls in each section header to change the display order. The new order is reflected immediately on the assessment page, and it also determines the order in Full PDF and print output.
Delete a section
In the section header, click the delete control.
Confirm the dialog. Matrix sections show Delete Matrix Section with a Delete Register button. Rich Text blocks show Delete Rich Text Block with a Delete Block button. Hazard Classifications, Hazardous Materials, Linked Documents, Image Gallery, and Map sections show their own delete confirmation. Image Gallery sections show Delete Image Gallery Section with a Delete Section button. Map sections show Delete Map Section with a Delete Section button. Linked Documents sections show Delete Documents Section with the linked document count. The button text changes to Deleting… while the action is in progress.
Deleting a Matrix Section removes its hazards from the assessment. Deleting a People & Groups section removes the participant list from the page. Deleting a Hazard Classifications, Hazardous Materials, Linked Documents, Image Gallery, or Map section removes that content block from the assessment. If you need the data back, you must add the section again and re-enter the contents.
Blank assessments and the start hub
When you create a blank assessment from scratch, the detail page opens to a start hub instead of an empty overview. Click Add Section, the Add Documents chip, the Add Image Gallery chip, or the Add Map chip to choose your first block and begin building the assessment.
Assessments created from a template or by duplicating an existing assessment copy the full section layout, including Matrix Sections, Rich Text blocks, People & Groups sections, Hazard Classifications sections, Hazardous Materials sections, Linked Documents sections, Image Gallery sections, and Map sections, together with their hazards and participants.
How section order affects exports
Full PDF and print output list sections in the same order they appear on the assessment page. If you have multiple Matrix Sections, the hazards in each register are grouped under that section’s title. Hazard Classifications sections print their selected pictograms and names, plus any classification notes. Hazardous Materials sections print each linked material with its name, description when present, and hazard pictograms. Linked Documents sections print each linked document with its name, identifier, version label, and a reference to locate the controlled copy in the document library. Image Gallery sections print as a titled block with a three-column thumbnail grid. Map sections print as a titled block with a static map snapshot when the section contains a pin, a custom title or description, or a non-default saved view; wholly empty defaults are omitted.
The Excel Workbook export includes an overview sheet, a People & Groups sheet when entries exist, and a data sheet for each Matrix Section. If the assessment has exactly one Matrix Section, the data sheet is named Risk Items. If there are multiple, each sheet is named after the section title. Rich Text blocks currently appear on the overview sheet in Excel; Full PDF and print include them as separate sections. Hazard Classifications, Hazardous Materials, Linked Documents, Image Gallery, and Map sections appear in Full PDF and print output. Map sections are included when they contain a pin, a custom title or description, or a non-default saved view; wholly empty defaults are omitted.
